If you have been in a motor vehicle accident, you may down load the Crash Report Form (pdf) located below. After downloading and completing the form mail one copy to the Registry of Motor Vehicles. A copy of the form must also be sent to the Police Department in the city or town where the crash occured. The Crash Report must be mailed within 5 days of the accident. If your crash happened in Carver mail a copy of your Operator Crash Report to:

You may also drop off a copy at the Carver Police Department.
If you need a copy of the Police Officers Crash Report for a Crash that occured in Carver, you must contact the Records Clerk. Please be advised that Crash Reports completed by Officers will not normally be available for up to 10 days after the date of the crash. Please contact the Records Clerk at 508-866-3465 with any questions or to request a report.
